The hospital can provide and/or organise a range of aids and services for those who need extra support at home. These will be discussed with you and your relatives, and planned for prior to, or soon after your admission.
Please make sure you discuss your discharge and future medical management with your doctor.
Please ask your doctor for medical certificates, relevant prescriptions and discuss follow – up treatment and appointments before you are discharged home.
You will be given written advice on current any changes to your medications treatment and follow-up arrangements at the time of discharge.
